Growth Group Covenant Form - group form (pdf)
Download the group covenant (leader's covenant). This form has a place for all members of the group to sign and once completed should be turned into the Growth Group office.

Growth Group Covenant Form - individual form (pdf)
Download individual covenant (member's covenant).  This form is for the individual members to fill out and take home.

Growth Group Feedback Form (pdf)
Download, print, fill it in and drop it off at the Growth Group Office.  Used to help our Growth Group staff and leaders and hosts to know how your Growth Group experience went, what went well, or any potential changes we should consider.
